SMART Family Literacy Liability and Media Release Waiver

Each participant who is 18 years old or older must complete their own waiver.

General Liability Waiver

I, the undersigned, assume all responsibilities for any accidents, injuries, or other harm, including illnesses, that happen to me while attending this SMART Family Literacy event. I hereby waive and release all other participants, the hosts, sponsors, partnering organizations, and SMART Family Literacy staff, directors, and officers from all claims and/or damage incurred in connection with this activity. 

Photo and Media Release

I, the undersigned, hereby grant SMART Family Literacy, Inc., all partnering organizations, sponsors and co-sponsors the unconditional right to use my name, voice, photographic likeness of myself (as well as my family members), images, and video in connection with any of their audio-visual productions, promotions, articles, educational publications, or press-releases. This permission extends to printed materials and electronic, including social media.
